Required school forms are mailed home to parents in late May/early June of each year.
Our School Forms Packet includes the following:
• School calendar
• Permission release forms
• Health and dental forms
• After-school program forms
• Parents’ Association registration and volunteer form
Please remember to inform the school of any changes to your contact information including your address, telephone numbers, and email addresses. Call the school office directly to make these changes. Additionally, remember to update your K12 Alerts account with any changes.
