Good Health
Prior to the beginning of each school year, parents must submit the required health/medical forms for their children: Health Appraisal Form, Medication Form, and Dental Health Form.
All health forms are sent home to parents in our annual information packet in late May/early June of each year. However, these are also available online (see below for downloads).
Please do not hesitate to contact our school nurse with any questions or concerns. We look forward to a happy and healthy school year.

Health Appraisals
• NYS education laws requires an annual physical exam for:
-New admissions
-Students in grades 2, 4, 7, and 10
-Participation in sports teams
-Triennially for the CSE
• Parents/guardians must sign back of form

• NYS law requires annual dental exams for:
-School entry, K, 2, 4, 7, and 10
• Annual exams can identify a condition that may interfere with a student’s ability to chew,
speak, or focus on school activities
